The Information We Collect and How We Use That Information

Self-Reported Data Generally. During your interactions with the Site, including when you provide any self-reported information, Company collects personally identifiable information or other nonpublic personal information from you. The type of self-reported information we may collect includes, without limitation: First and Last Name; City; State; Address; Zip Code; Work Phone; Home Phone; Best Time to Call and Email Address, as well as (collectively, “Personal Information”). We may also collect aggregate, anonymous, statistical data, such as usage, traffic patterns and user demographics based on your Personal Information (“Anonymous Information”). Self-Reported Medical Data. During your interactions with the Site, upon prompting, you may be asked to provide UPIN and DEA numbers in order to receive prescription samples and other benefits from Vendors (collectively, “Nondisclosed Information”). Company shall not disclose Nondisclosed Information to Vendors, and shall restrict Company’s use of Nondisclosed Information to the verification of your identity. 

Electronically Produced Data.

Cookies. Company may use cookies and other tools to collect information about how you use the Site (“Cookie Information”). Any information you transmit online in such forums will be made public and Company cannot control nor is it responsible for the use any third party may make of such information.
